Het VPT is een leveringsvorm: de manier waarop u zorg ontvangt.<\/p>\n<p>Bij een VPT blijt u thuis wonen en levert een zorginstelling het volledige pakket zorg. Het is dezelfde zorg die. U zou krijgen in een zorginstelling. De zorginstelling waar u de zorg van ontvangt, heeft een contract met het zorgkantoor. Het zorgkantoor betaalt de zorginstelling rechtstreeks voor de geleverde zorg.
